I wish we had cool little narrow hidden away walkways and shops like that here in Pittsburgh. Its one of our oldest cities… but definitely not old enough to have that kind of architecture. Maybe off by a millennia

They are blatantly paid by the restaurants to bring them tourists and they get a percentage from your order. (Not the guides themselves, the owner). Walk around yourself. We, trasteverians, are friendly bunch, so just ask and we'll be happy to help.
